Abstract

Introduction: Loneliness among the elderly is a growing public health concern associated with adverse physical and mental health outcomes. In Malaysia, evidence on loneliness, particularly within Federal Land Development Authority (FELDA) communities, remains limited. This study aimed to determine the prevalence of loneliness and its independent predictors among elderly residents in FELDA in Kuantan, Pahang. Methods: A cross-sectional study was conducted among 370 individuals aged ≥60 years from five selected FELDA areas using simple random sampling. Data on sociodemographic, socioeconomic, and medical characteristics were collected through structured questionnaires. Descriptive statistics and Chi-square tests assessed bivariate associations, while multivariable logistic regression identified independent predictors. Variables with p < 0.25 in univariate analysis and clinically important factors were included in the regression. Results: The prevalence of loneliness was 70.6%, with low (56.6%), moderate-to-severe (39.7%), and intense (3.8%) levels. Emotional loneliness was most common (56.6%), followed by mixed (34.7%) and social loneliness (9.2%). Multivariable analysis showed that living alone significantly increased the odds of loneliness (AOR = 5.30; 95% CI: 1.14–24.57; p = 0.033). Higher educational attainment was protective, while multiple comorbidities were associated with increased odds (AOR = 1.76; 95% CI: 1.00–3.09; p = 0.050). Other factors were not significant. Conclusion: Loneliness is highly prevalent among elderly residents in FELDA communities. Targeted interventions focusing on social support, educational engagement, and chronic disease management are crucial to mitigate loneliness and improve wellbeing among the elderly.
