Adjunctive relaxation modalities (deep breathing and murottal therapy) for pain and hypertension control in non-ST-elevation myocardial infarction (NSTEMI): a case study

Abstract

Background: Non-ST-elevation myocardial infarction(NSTEMI) with severe hypertension increases cardiac workload and mortality risk. Alongside guideline-directed pharmacotherapy, non-pharmacological interventions that modulate autonomic balance can help stabilise hemodynamics and provide analgesia. Case Presentation: A77-year-oldmalepresentedwithseverechestpain(Numeric Rating Scale [NRS] 7/10), shortness of breath, diaphoresis, and acute hypertension (201/100 mmHg). Diagnostics confirmed NSTEMI via inferior lead ST-segment depression, T-wave inversion, and highly elevated troponin I (40,065 pg/mL). The patient was immediately treated with standard pharmacotherapy, including fondaparinux, dual antiplatelets, atorvastatin, perindopril, and pantoprazole. Concurrently, we administered structured adjunct nursing interventions combining slow diaphragmatic deep breathing with Murottalaudiotherapy (Surah Ar-Rahman). Over a 24-to 48-hour period, the patient's pain decreased to 4/10, blood pressure stabilised at 152/95 mmHg, and heart rate dropped from 65 to 58 bpm without adverse hypotension. Conclusion: Combining diaphragmatic deep breathing and Murottal audiotherapy is an effective, non-invasive adjunct nursing intervention. When integrated with standard medical care, this approach safely supports pain reduction, anxiety alleviation, and hemodynamic stability in hypertensive NSTEMI patients.
