Abstract

Introduction: Mortality in the Intensive Care Unit (ICU) is strongly associated with severity and progression of organ failure. The Sequential Organ Failure Assessment (SOFA) score is a common and validated scoring system to evaluate and quantify organ failure in critically ill patient. SOFA score has a strong association with ICU and in-hospital mortality. However, to the best of our knowledge, the associationship of SOFA score and 1year mortality was not well established. We evaluated the predictive utility of serial SOFA with 1-year mortality in critically ill patient Materials and method: This was a retrospective observational cross sectional study using the existing record of patients admitted to general ICU Sultan Ahmad Shah Medical Centre from the 1st June 2017 to the 30th May 2018. Data was collected from patients’ daily clinical charts and medical records. SOFA score on day-1 until day-3 and on discharge were recorded and subsequently delta SOFA was calculated. Result: A total of 120 patients’ data were collected. There was a significant association between the day-1 to day-3 and on the discharged SOFA score with 1-year mortality. There was also significant relationship for delta SOFA day-1 to discharge and delta SOFA day-3 to discharge with the outcome of the 1-year mortality. SOFA score on discharge and delta SOFA day-3 to discharge have the highest AUC value, hence best predicted 1-year mortality. Cardiovascular and renal were the most significant individual component of SOFA score that contributed to 1year mortality. There was a strong correlation between NUTRIC score and admission category with 1-year mortality. Conclusion: The study demonstrated that there was a significant correlation between SOFA score and 1-year mortality in critically ill patients. 1-year mortality increased with increased serial daily SOFA and delta SOFA. Among the individual components of SOFA score, cardiovascular and renal were the most significant component that contributed to 1year mortality. The NUTRIC score and admission category were the independent factors that associated with 1-year mortality in critically ill patients.
