Abstract
Background: Management of hypertension in young individual is challenging due to the lack of cardiovascular disease (CVD) risk assessment tools specific to this age group. The 10-year risk of developing acute myocardial infarction (AMI) among young individuals with hypertension is more likely to be underestimated owing to young age is generally considered as a protective factor. Thus, identification of novel biomarker(s) to improve risk stratification of AMI in young hypertensive individuals is essential for more accurate AMI prediction in this cohort. This study investigated plasma and urinary haptoglobin concentration in young AMI to evaluate its potential as a biomarker in the establishment of AMI in young hypertensive individuals. Methods: This is a cross-sectional study that recruited 40 male adults aged between 18 to 45 years in control, hypertensive and AMI group respectively. Blood samples were collected from all subjects while urine samples were collected from AMI patients and controls. Plasma and urinary concentrations of haptoglobin were measured using enzyme-linked immunosorbent assays. Other CVD risk factors including high sensitivity C-reactive protein (hs-CRP) levels were analyzed. Findings: The mean age for AMI patients was 37.4±5.0 years, compared to 35.2±6.8 and 33.1±5.3 years for hypertensive and control group respectively. Plasma concentration of haptoglobin in the AMI group was the highest as compared to hypertensive and control group (290.63±99.90 vs. 208.47±112.93 vs. 170.02±108.11 ng/ml, p<0.006). Plasma haptoglobin concentration was significantly associated with AMI independent of other known CVD risk factors (OR: 0.985, 95% CI 0.973-0.997, p=0.017) and positively correlated with hs-CRP (r=0.0370, p<0.001). Urinary excretion of haptoglobin was higher in AMI patients than the controls, yet there was no significant difference. Conclusion: Plasma haptoglobin is a potential biomarker to identify young hypertensive individuals who are at risk of developing premature AMI, while the role of haptoglobin in urine sample was otherwise inconclusive.
