Abstract

Background: Specimen collection procedure is a medical procedure that is frequently used in clinical settings. The procedure requires close interaction with a variety of patients, some of whom are cooperative and others who are not. Therefore, nurses are required to possess not only technical skills (hard skills) but also soft skills to handle patient care in the clinical setting. Looking into the current structured clinical skill assessments on specimen collection procedures, it places too much emphasis on technical skills and soft skills assessments are not clearly seen. Objective: This study aimed to examine the soft skill elements in objectively structured clinical skill assessment on specimen collection procedure in the undergraduate nursing programme. Method: This study applied a clinical audit on the retrospective structured clinical skill assessment exam questions by using the clinical skill assessment index VII (CSAI-VII) instrument. A total of 5 specimen collection procedures set of exam questions were assessed. The exam questions included clinical case scenarios, procedure checklist, examiner’s comments, and documentation charts of specimen collection procedures. Result: The 5 specimen collection procedures found in this study were; collecting a urine specimen for culture and sensitivity test 3 (60%), collecting a sputum specimen for culture and sensitivity test 1 (20%), and others (Vulva swabbing test) 1 (20%). Although 5 types of specimen collection procedures for the client were identified in this study, some of the procedures did not find in the collected OSCE documents such as collecting blood specimens and collecting stool specimens. Regards to the soft skill elements for the specimen collection procedure, there were 9 elements of soft skills were identified related to this procedure: communication skills (CS), social skills and responsibilities (SSR), critical thinking skills (CTS), Problem-solving skills (PSS), teamwork skills (TS), leadership skills (LS), professional and ethical decision-making skills (PEDMS), numeracy skills (NS), and interpersonal skills (IS). It was found that all of the OSCE documents of the specimen collection procedure skill assessment checklist/rubric have the CS 5 (100%). However, other soft skill elements were not found in this specimen collection procedure’s checklist/rubric. Implications/Conclusion: It was discovered that a variety of aspects of the structured clinical skill assessment on specimen collection procedure, including the type of specimen collection, the nature of the clinical scenario, and the elements of the soft skill checklist, could be improved. Thus, having a well-structured clinical skill assessment exam question is essential to producing holistic graduate nurses who are equipped with hard skills and soft skills for the current healthcare delivery system.
