Prevalence of metabolic syndrome and its associated factors among registered transgender women on cross sex hormonal therapy in Kuantan, Pahang

Abstract

Metabolic syndrome is a constellation of interrelated risk factors of metabolic origin that could eventually lead to cardiovascular disease in the long run. Hypothetically, hormonal therapy has been linked to metabolic syndrome in many studies particularly in the transgender community. Despite the lack of evidence to showcase the potential adverse effect of long-term hormonal therapy in transgender population, the existence data on the relationship between hormonal therapy and cardiovascular mortality and morbidity cannot be ignored. Hence, this study is aimed to assess the prevalence of metabolic syndrome and its associated factors among transgender women on cross sex hormonal therapy. A cross-sectional study was conducted among 90 transgender women around Kuantan district in Pahang. A self-dministered validated Malay long version of International Physical Activity Questionnaire (IPAQ-M) were used to assess the physical activity. The statistical analysis was done using the chi-squared test, which then proceeded with multiple logistic regression. Majority of the respondents were aged below 40-year�old (57.8%), Malay ethnicity (94.4%) and Islam in religion (95.6%). Factors that contributed to risk of metabolic syndrome in the study include age (OR = 1.063 CI = 1.013,1.116, p = 0.012), consumption of patch containing combination of ethynyl oestradiol progesterone (OR = 17.036 CI = 1.585,183.048, p = 0.019), known case of hypertension (OR = 0.203 CI = 0.005,0.817, p = 0.025) and diabetes (OR = 0.018 CI = 0.001, 0.252, p = 0.003). About 39% of the respondents (n = 35) were detected to have metabolic syndrome based on Joint Interim Statement criteria for which about half of those diagnosed with metabolic syndrome are above 40 years old (n = 18). As a conclusion, transgender women are greatly at risk of getting metabolic syndrome. Intensive and effective intervention to curb the occurrence of metabolic syndrome should also targeted at this minority population.
