Abstract
Advances in artificial intelligence (AI) are beginning to influence many areas of medicine, including how clinicians assess and manage patients nearing the end of life. These technologies can provide insights into prognosis, risk stratification, and treatment pathways, yet they also raise complex ethical questions about how decisions should be made when human judgement intersects with algorithmic output. End-of-life care is especially sensitive because choices are deeply personal, shaped by a patient’s values, beliefs, and understanding of what it means to live and die well. Traditionally, ethical decision-making in this context draws on core principles such as autonomy, beneficence, nonmaleficence, justice, and the sanctity of life, which help clinicians and patients navigate the tension between extending life, avoiding harm, and respecting personal choices. This paper employs qualitative research methods to examine the ethical and legal dilemmas introduced by AI-assisted clinical insights. It deliberates on how these dilemmas should be addressed through thoughtful harmonisation of existing ethical and legal standards on end-of-life care with Shariah principles within the Malaysian context. By bringing together longstanding bioethical values and culturally meaningful moral principles, the study proposes a preliminary ethico-legal framework to help interpret and use AI in ways that respect human dignity and support shared decision-making. Its contribution lies in suggesting how such framework might be applied to emerging AI-related challenges and in outlining normative directions for the responsible integration of AI into end-of-life care.
