Abstract
Why do medical errors occur so frequently when many of our healthcare workers are highly trained and dedicated people? Is the problem due to 'bad' workers or something more complex? This talk discusses the opposing views on why medical errors occur, with emphasis on the importance of using a systems thinking in managing medical errors.
