Abstract

Background: Prehypertension is associated with increased risk of cardiovascular disease. Chronic inflammation plays a role in the pathophysiology of essential hypertension, with epigenetic dysregulation involvement. However, its role in prehypertensive state is not known. The aim of this study was to investigate the association between DNA methylation level of IL-6 promoter in prehypertensive (PreHT) and normotensive (NT) young adults. Method: We recruited 80 NT and 80 PreHT healthy subjects age 18-45 years in Kuantan, Pahang in an observational cross-sectional study. We analysed the DNA methylation level of IL-6 of peripheral leukocytes using bisulphite conversion and MethyLight assay. The difference in IL-6 promoter methylation level between the two groups was analysed. Result: The mean age of NT was 30.8 (6.60) years and PreHT was 31.3 (6.9) years. There was no statistical difference in age between the two groups (p=0.655). The mean blood pressure was 110(8)/73(5) mmHg in NT and 125(7)/82(5) mmHg in PreHT. The IL-6 promoter interleukin-6methylation level was significantly lower in preHT compared to NT subjects (p<0.001). Conclusion: Our study indicates that hypomethylation of IL-6 promoter was associated with preHT in young adults. IL-6 methylation could prove to be an important indicator for future hypertension in these subjects. Gene expression study and longitudinal study are warranted to establish the methylation effect on IL-6 expression over time.
